Mangaluru: A 28-year-old male presented to Urology OPD at Yenepoya Medical College Hospital, Deralakatte with gradual loss of balance while walking, headache and giddiness. He was found to have severe hypertension and after admission, was diagnosed to have a 14 cms tumor in the right adrenal gland, which is located above the kidney. The tumor was large, encasing the adjacent large vessels and hence risky to operate. His BP was uncontrollable even with three antihypertensive drugs and his condition deteriorating.
The urology team in association with anesthesia, medicine, and well-established cardiology and the vascular team prepared the patient and he underwent da Vinci robot-assisted surgical removal of this large tumor, which was involving the vena cava, the largest vessel taking the blood from different parts of the body to the heart. The blood pressure returned to normal once the blood vessel that was connecting the tumor to the vena cava, that was pouring in the hormones was ligated. The tumor could be removed completely without any complications. All antihypertensive medications were stopped after surgery and the patient discharged in stable condition.
This is one of the rare cases that occur and a multidisciplinary team involving urologists Dr. Mujeeburahiman, Dr. Nischith Dsouza and Dr. Altaf Khan, the anesthesia team lead by Dr. Harish Hegde, intensivist, a cardiologist and vascular surgeon carried out this successful procedure, using state of the art equipment at Yenepoya Medical College Hospital. Mr. Yenepoya Farhaad, Pro-Chancellor, and Dr.M Vijayakumar Vice Chancellor have agreed to provide high tech robotic surgery for the needy patients at a concessional rate in future too. Mr. Nelvin Nelson coordinated the entire procedure.
