‘Diabetes, heart disease increases risk of benign prostatic hyperplasia’

Mysuru: Diabetes, as well as heart disease, increases the risk of Benign prostatic hyperplasia (BPH), which is also called prostate gland enlargement, said Dr TP Dinesh Kumar, Consultant Urologist, Columbia Asia hospital, Mysuru

“The disease is common as men get older. By the age of 60, approximately 50 percent of men will have some signs of BPH and by the age of 85, almost 90 percent of men usually show signs of this condition. Enlargement of the prostate gland often causes uncomfortable urinary symptoms such as urine flow blockage. It also causes bladder, urinary tract and kidney ailments. Certain symptoms associated with prostate gland enlargement are frequent or urgent need to urinate, increased frequency of urination at night, difficulty in urinating, weak urine stream and stream that stops and starts, dribbling at the end of urination, inability to completely empty the bladder and blood in urine. Having a blood relative such as a father or brother with prostate problems means you are more likely to have problems,” added Dr Dinesh Kumar.

He continued, “If you neglect prostate enlargement, the complications you can develop are a sudden inability to urinate or urinary retention, urinary tract infection such as pain during urination, recurring need to urinate. Retention of urine can cause minerals, such as calcium or magnesium salts, to crystallize and form bladder stones. A bladder that has not emptied completely can stretch and weaken over time causing bladder damage. The pressure in the bladder from urinary retention can also directly damage the kidneys. The size of the prostate does not necessarily determine the severity of your symptoms. Medications are the most common treatment for mild to moderate symptoms. Minimally invasive therapy is recommended if your symptoms are moderate to severe. For an enlarged prostate, Holmium Laser prostate surgery is an invasive treatment. The procedure of HOLEP uses a laser to remove tissue that is blocking the urine flow. It removes the entire portion of the prostate that is blocking the urine flow. The treatment provides a more lasting solution for men with enlarged prostate. HOLEP offers faster recovery and symptom relief when compared with traditional prostate surgery.”

HOLEP has a few other advantages over TURP such as the amount of bleeding is often lesser, shorter hospital stay and faster recovery (about 1-2 days). The chances of recurrence requiring further surgery is very low. It is also safer for patients who are on blood thinners like aspirin, warfarin as they can be operated with minimal blood loss. Patients who are very old and suffering from heart disease, patients on pacemaker, obese, asthmatic, COPD, diabetics, prone to electrolyte imbalance can safely undergo HOLEP.
