Strong bones are built and maintained over many years, but everyday habits, nutritional deficiencies and certain medical conditions can gradually reduce bone strength. While some decline in bone density is a natural part of ageing, factors such as poor nutrition, physical inactivity, smoking and excessive alcohol consumption can accelerate bone loss and increase the risk of osteoporosis and fractures.

Low calcium and vitamin D intake, prolonged sitting, restrictive diets and certain medicines or health conditions can all affect the skeletal system. Many of these risk factors, however, can be modified through timely lifestyle changes and medical guidance.
1. Not getting enough calcium and vitamin D
Calcium is one of the primary minerals required to maintain strong bones, while vitamin D helps the body absorb calcium efficiently.
A diet that regularly lacks calcium-rich foods can gradually affect bone health. Dairy products, leafy green vegetables and fortified foods can contribute to daily calcium intake.
Vitamin D deficiency can also become a concern among people who have limited exposure to sunlight or do not obtain sufficient vitamin D through their diet.
Over time, inadequate levels of these nutrients can contribute to reduced bone density and make bones more vulnerable to fractures.
2. Sitting for too long
Bones, like muscles, respond to physical activity. Weight-bearing exercises place healthy stress on bones and encourage the body to maintain bone strength.
Walking, jogging, dancing, climbing stairs and resistance exercises are examples of activities that can support bone health.
On the other hand, spending most of the day sitting and having little physical activity can contribute to bone loss. This becomes particularly important with increasing age, when bone density naturally tends to decrease.
Breaking up long periods of sitting and incorporating regular movement into the day can therefore be beneficial.
3. Smoking
Smoking is another significant risk factor for poor bone health.
Tobacco can interfere with the activity of cells involved in bone formation and may also affect the body’s ability to use calcium effectively. Long-term smoking has been associated with weaker bones and an increased risk of fractures.
Quitting smoking can therefore have benefits extending beyond cardiovascular and respiratory health. Protecting bone health is another reason to avoid tobacco use.
4. Excessive alcohol consumption
Regularly consuming excessive amounts of alcohol can negatively affect bone health.
Alcohol can interfere with vitamin D metabolism and may contribute to poor dietary habits, potentially reducing the intake of nutrients required for healthy bones.
There is another concern for people who already have weak bones. Excessive alcohol consumption can increase the likelihood of falls, which can subsequently raise the risk of fractures.
Limiting alcohol consumption is therefore an important part of maintaining overall skeletal health.
5. Too much salt and sugary drinks
Everyday dietary choices can also influence bone health.
A diet high in sodium can increase calcium loss through urine. Consistently consuming excessive amounts of salty, highly processed foods may therefore make it more difficult to maintain adequate calcium levels.
Sugary carbonated drinks can also become a concern when they regularly replace water, milk or other more nutritious beverages and contribute to an overall diet lacking essential nutrients.
A balanced diet containing adequate calcium, protein, vitamins and minerals is preferable to relying heavily on highly processed foods and sugary drinks.
6. Being underweight or following fad diets
Being significantly underweight can affect bone health because the body may not receive adequate nutrients required to maintain skeletal tissue.
Restrictive or fad diets can be particularly problematic if they provide insufficient protein, calcium, magnesium or vitamin K.
Rapid weight loss and repeatedly following highly restrictive diets can therefore have consequences beyond changes in body weight.
At the same time, being overweight or obese can also indirectly increase fracture risk in certain circumstances. Maintaining a healthy and sustainable body weight is generally more beneficial than repeatedly pursuing rapid weight-loss programmes.
7. Certain medicines
Some medicines required to manage other health conditions can affect bone density when used for prolonged periods.
Long-term corticosteroid treatment is a well-established risk factor for bone loss. Certain anti-seizure medicines and some treatments used for cancer can also influence bone health.
People who require these medicines for extended periods should not stop or alter their treatment without medical advice. Instead, they should discuss their individual bone-health risk with their doctor.
Depending on the person’s age, medical history and medication, a doctor may recommend preventive measures or bone-density assessment.
8. Medical conditions and hormonal changes
Several medical conditions can contribute to accelerated bone loss.
These include thyroid disorders, rheumatoid arthritis, celiac disease and chronic kidney disease. Such conditions can affect the body’s ability to absorb nutrients, regulate minerals or maintain healthy bone metabolism.
Hormonal changes are another important factor.
Bone loss can accelerate after menopause because of declining oestrogen levels. As a result, postmenopausal women have an increased risk of developing osteoporosis.
Identifying and managing underlying medical conditions early can therefore play an important role in protecting bone strength.
How to keep bones strong
Protecting bone health does not depend on a single food or exercise. Instead, it involves maintaining healthy habits consistently over time.
A balanced diet containing adequate calcium and protein can provide the nutrients required for maintaining bones. Sufficient vitamin D is also important for calcium absorption.
Regular weight-bearing and resistance exercises can help maintain bone mass, while avoiding smoking and limiting excessive alcohol consumption can reduce additional risk factors.
Maintaining a healthy body weight is also important, particularly because being significantly underweight can negatively affect skeletal health.
Who should consider bone-density screening?
Some people may benefit from discussing bone-density screening with their doctor.
Dr Garg said adults over 50, postmenopausal women and people with other risk factors should discuss their individual need for bone-density assessment with a physician.
Early identification of reduced bone density can allow preventive measures to be introduced before a serious fracture occurs.
Bone health is therefore something that should not be considered only in old age. The habits developed earlier in life can influence skeletal strength later on.
By maintaining a nutritious diet, staying physically active, avoiding tobacco and excessive alcohol, and addressing medical or hormonal risk factors, people can take practical steps towards protecting their bones and preserving mobility and independence as they age.
