New Delhi: The monsoon season brings a sharp rise in mosquito-borne illnesses such as dengue, malaria and chikungunya across India. Since these infections often begin with similar symptoms, doctors say laboratory testing at the right time is essential for an accurate diagnosis and timely treatment.

High fever, chills, headache, weakness, body aches and joint pain are common to all three diseases, making it difficult to distinguish one from another based on symptoms alone. Health experts advise people not to rely on self-diagnosis or self-medication, but to seek medical evaluation and undergo appropriate tests depending on the stage of the illness.

Similar symptoms can be misleading

Dengue, malaria and chikungunya frequently present with overlapping symptoms during the initial days of infection.

Patients may experience:

  • High fever
  • Chills
  • Headache
  • Body ache
  • Joint pain
  • Fatigue and weakness

Because these symptoms are common to several viral and parasitic infections, doctors emphasise that clinical examination should be supported by laboratory testing.

Early diagnosis not only helps ensure proper treatment but also reduces the risk of complications.

Dengue: Right test depends on the day of illness

During the first week of fever, the Dengue NS1 antigen test and Dengue RT-PCR are considered the most effective diagnostic tools.

RT-PCR detects the virus’s genetic material, while the NS1 antigen test identifies an active dengue infection during its early phase.

The Dengue IgM antibody test becomes more reliable after five to seven days, once the immune system has started producing antibodies.

Doctors caution that:

  • An IgM test performed too early may produce a false-negative result.
  • A negative NS1 or RT-PCR later in the illness does not completely rule out dengue.

A Complete Blood Count (CBC) is also routinely advised to monitor platelet count, white blood cells and haematocrit.

However, experts stress that platelet count alone should not be used to diagnose dengue. The patient’s hydration status, blood pressure, bleeding tendency and overall clinical condition are equally important.

A rising Immature Platelet Fraction (IPF) may indicate recovery of platelet production but should only be interpreted alongside other clinical findings.

Patients are advised to avoid medicines such as aspirin and ibuprofen if dengue is suspected because they can increase the risk of bleeding. Paracetamol is generally recommended for fever relief.

Chikungunya: PCR most useful during early infection

Chikungunya commonly causes severe joint pain, often accompanied by stiffness and swelling.

Unlike many viral fevers, the joint pain may continue for weeks or even months after the fever subsides.

During the first week, Chikungunya RT-PCR is the preferred test because it directly detects the virus.

After approximately one week, the Chikungunya IgM antibody test becomes more useful.

As with dengue, timing is important because antibody tests performed too early may yield false-negative results, while IgM antibodies can remain detectable even after recovery.

Malaria requires immediate testing

Doctors recommend testing for malaria as soon as the disease is suspected, particularly when fever is accompanied by chills.

The standard diagnostic methods include:

  • Peripheral blood smear
  • Rapid malaria antigen test

A blood smear also helps determine the species of malaria parasite and estimate parasite levels.

Experts note that a single negative test does not always rule out malaria, especially during the early stages when parasite levels may be low.

If symptoms continue and clinical suspicion remains high, repeat blood smears may be necessary.

Although Malaria PCR can help detect low parasite levels and identify the exact species, it should not delay immediate smear or antigen testing in acutely ill patients.

Avoid self-medication

Doctors strongly advise against starting treatment solely based on symptoms.

Malaria requires specific antimalarial drugs, while dengue and chikungunya are viral infections that are generally managed with hydration, fever control and close monitoring.

Using leftover antibiotics without medical advice does not treat viral infections and may contribute to antibiotic resistance while increasing the risk of side effects affecting the kidneys or liver.

Similarly, antimalarial medicines should not be started without proper medical evaluation wherever possible.

High-risk groups need early medical attention

Some people are more vulnerable to complications and should seek medical care without delay.

These include:

  • Infants and young children
  • Pregnant women
  • Elderly people
  • Patients with diabetes
  • Individuals with kidney or heart disease
  • Cancer patients
  • People with weakened immunity

Older adults may not always develop very high fever. Instead, confusion, excessive weakness, poor appetite, drowsiness or reduced urine output may be early warning signs.

Immediate medical attention is essential if a patient develops:

  • Persistent vomiting
  • Severe abdominal pain
  • Bleeding
  • Breathlessness
  • Fainting
  • Confusion
  • Seizures
  • Reduced urine output
  • Sudden deterioration after the fever begins to subside

PCR testing plays an important role

PCR-based tests are particularly valuable during the early stage of infection, before antibodies develop.

They are also useful when:

  • Routine tests are negative despite strong clinical suspicion
  • More than one infection is suspected
  • Mixed infections need to be ruled out

Health experts emphasise that monsoon fevers should never be diagnosed based on symptoms alone. Choosing the appropriate test at the correct stage of illness, along with careful clinical assessment, remains the best approach for ensuring accurate diagnosis and effective treatment.
