Mangaluru: In a remarkable medical achievement, doctors at Father Muller Medical College successfully performed a rare autologous amniotic patch infusion using the advanced AmnioSeal technique on a 40-year-old woman carrying triplets.

The patient was referred for a detailed anomaly scan, which revealed a dichorionic triamniotic triplet pregnancy — including one monochorionic pair sharing a placenta. A day later, one of the monochorionic sacs showed signs of a premature rupture of membranes (PV leak), posing a significant risk of pregnancy loss.

Despite conservative management, the fluid levels in the affected sac dropped to dangerously low levels. To save the pregnancy, the medical team proposed an innovative regenerative therapy: an autologous amniotic patch infusion aimed at sealing the leak and minimizing immune reaction.

This complex and rare procedure involved harvesting the patient’s own blood platelets through apheresis. These were then infused with cryoprecipitate directly into the amniotic sac. The intervention was carried out with extreme precision by the Fetal Medicine team, led by Dr Muralidhar G.K, Dr Ram Shenoy, and Dr Prathima Prabhu, in coordination with obstetricians Dr Sujaya V Rao and Dr Joylene D’Almeida.

Overcoming challenges like minimal fluid, fetal movements, and procedural risks, the team managed to restore amniotic levels, prevent infection, and stabilize the fetuses. Post-procedure monitoring indicates a positive trajectory.

“This is a milestone in regenerative fetal treatment,” said Dr Ram Shenoy, noting its rarity in triplet pregnancies. The patient remains under careful observation.
