Mangalore, 30 October: KMC Hospital has successfully treated a 72-year-old patient, Mahesh (name changed) suffering from a large, long-standing multinodular goitre that had become symptomatic and started causing compressive symptoms. Despite the challenges posed by the patient’s age, multiple co-morbidities, and surgical risks, the team of specialists ensured a safe and successful outcome. The surgery was led by a multidisciplinary team including Dr. Avinash K (Laparoscopic & General Surgeon), Dr. Manohar Pai (General Surgeon), and anesthetists Dr. Sunil and Dr. Frieda, KMC Hospital Mangalore whose combined expertise ensured the patient’s safety throughout the complex procedure.
The patient presented with a visibly prominent goitre but fortunately showed no signs of invasion or suspicious lymphadenopathy. What made the case complex was not just the size of the thyroid growth but also the patient’s medical background. Advanced age, a difficult airway, and multiple co-morbidities including cerebrovascular disease, hypertension, benign prostatic hypertrophy, and diabetes significantly increased both anesthesia and surgical risks. The tumour size measured 18×12 cm.

The surgery was technically demanding due to the size of the goitre, distorted anatomy, dense adhesions, and its proximity to important neurovascular structures. Limited neck extension further added to the complexity. However, with meticulous planning and execution, the procedure was successfully carried out. The patient was safely discharged on the fifth postoperative day.
