Government Directs Hospitals: No ICU Admission for Critically Ill Patients

Intensive Care Unit Admission: The recent directives issued by the Union Health Ministry regarding Intensive Care Unit (ICU) admissions emphasize key guidelines for hospital protocols. According to the recommendations compiled by a panel of 24 experts, critically ill patients may not be admitted to the ICU if they or their relatives refuse such admission. The guidelines stress that ICU resources should be prioritized for cases where treatment is likely to have a significant impact on survival.

Living wills or advanced directives against ICU care are to be respected, and patients falling under these categories should not be admitted to the ICU. In situations where resources are limited, such as during a pandemic or disaster, low-priority criteria should be considered for ICU admission.

The criteria for ICU admission are specified to include organ failure, the need for organ support, or anticipation of medical deterioration. This encompasses factors like altered consciousness, hemodynamic instability, respiratory support needs, acute illnesses requiring intensive monitoring, and patients undergoing major surgery or experiencing intraoperative complications.

On the other hand, the guidelines highlight scenarios where critically ill patients should not be admitted to the ICU. These include informed refusal by the patient or their next of kin, diseases with established treatment limitations, individuals with living wills or advanced directives against ICU care, terminally ill patients with a medical judgment of futility, and low-priority criteria during resource-limited situations like pandemics or disasters.

The criteria for ICU discharge include the return of physiological parameters to near-normal or baseline status, reasonable resolution and stability of the acute illness, and agreement from the patient or family for discharge based on treatment-limiting decisions or palliative care.

The guidelines also stress the importance of monitoring various parameters, such as blood pressure, pulse rate, respiratory rate, breathing pattern, heart rate, oxygen saturation, urine output, and neurological status, in patients awaiting an ICU bed.
