September 24, Mangalore: A team of specialists at KMC Hospital, BR Ambedkar Circle, successfully managed a highly complex case involving a 38-year-old woman from Payyanur suffering from “placenta percreta,” a rare but serious pregnancy complication. The patient, who had undergone three previous cesarean sections, faced life-threatening risks in her fourth pregnancy due to the abnormal invasion of the placenta into the uterine muscle and surrounding organs, such as the bladder.

Diagnosed in the second trimester, the patient was referred to KMC Hospital from a private hospital in Payyanur after experiencing severe pain and bleeding at eight months of pregnancy. The multidisciplinary team, led by Dr. Vidyashree Kamath (OBG specialist), Dr. Sanman Gowda (urologist), Dr. Mario Bucello (neonatologist), and Dr. Madhusudan Upadhyay (anesthesiologist), worked together to ensure the safe delivery and recovery of both the mother and baby.

Managing a High-Risk Pregnancy

Dr. Vidyashree Kamath, who led the obstetric team, explained that “placenta percreta” occurs when the placenta penetrates the uterine wall and sometimes even extends to nearby organs, such as the bladder. “Cases of ‘placenta percreta’ have increased ten-fold in the last decade due to the rise in cesarean deliveries. It is a major cause of maternal morbidity and mortality and often necessitates a hysterectomy, leading to severe bleeding, massive blood transfusions, ICU admissions, prolonged hospitalization, and sometimes loss of fertility,” Dr. Kamath noted. She emphasized that early detection, meticulous delivery planning, and a multidisciplinary approach with access to blood bank facilities were crucial in managing such cases.

In this particular case, at 32 weeks of gestation, the patient’s placenta had invaded part of her bladder, causing severe abdominal pain and bleeding. “An immediate delivery was necessary to save the lives of both the mother and the baby. The team first managed the bladder involvement and controlled the bleeding before performing a cesarean section,” Dr. Kamath explained. Post-surgery, the patient received adequate blood and blood products and was closely monitored in the ICU before being discharged after a week of hospitalization.

Critical Role of Multidisciplinary Expertise

Urologist Dr. Sanman Gowda highlighted the challenges of the surgery, explaining that a pre-operative examination revealed the placenta’s invasion into the bladder. “Following the successful delivery, we carefully repaired the bladder, which required precision to manage the high risk of bleeding,” Dr. Gowda said. A post-surgery cystoscopy confirmed the successful repair of the bladder.

The premature baby, born at 31 weeks with a low birth weight of 1.76 kg, was immediately moved to the Neonatal Intensive Care Unit (NICU) under the care of Dr. Mario Bucello. “Thanks to our specialized NICU team, the baby made a full recovery and was discharged after 15 days,” Dr. Bucello reported.

Dr. Madhusudan Upadhyay, the anesthetist, played a critical role in managing the patient’s condition throughout the procedure.
